Job Description

We are seeking a Registered Nurse who will function as a per diem field RN Supervisor position for our Personal Care division.

The per diem field RN Supervisor will be responsible for performing field visits for admission and ongoing supervision of personal care aides for patients requiring these services.

Responsibility of the per diem RN Supervisor:

  • Understands regulations for Licensed Home Care Agencies
  • Perform initial and follow-up patient assessments to identify changes in patient condition and development of an appropriate care plan for personal care aide services.
  • Coordinate with director of administrative services to ensure timely completion of patient assessment and aide supervisory visits.
  • Cooperate with quality assurance and compliance departments to facilitate development and improvement of services provided to patients.
  • Supervision of paraprofessional personnel to ensure provision of high-quality services.
  • Case management of all personal care division patients through engagement of patient and/or family in addressing patient care needs.
  • Periodic home visits for evaluation, re-evaluation of patient status, recertification, and revision of care plan to meet the needs of the patient.
  • Timely completion of required electronic health record documentation at the point of care.
  • Participation in Quality Assurance Activities as directed.
  • Cooperate with and participation in risk management activities include incident and complaint investigation and resolution as required.
  • Ensure compliance with Infection Control and Safety Policies and Procedures
  • Identification of educational needs with utilization of training resources and/or development of training program as required.
