Job Description

The Intake Coordinator will directly educate a prospective patient through the intake process leading to enrollment with Always Compassionate Home Care. This person will work closely with referral sources, account managers, personal care team, and provider relations to ensure all tasks are efficiently completed associated with the enrollment process. The Intake Coordinator is expected to maintain ethical conduct, compliance with state, federal and agency regulatory requirements, and will be responsible for quality and satisfaction of prospective patients and referral sources. This person will focus on intake for all private pay referrals, client satisfaction, and working closely with coordination team to enroll and provide staffing for prospective patients.
SALARY: starting at $55,000

Qualifications:

  • At least one year of Intake experience in a Home Care setting
  • Ability to work in a fast-paced environment
  • Excellent communication skills
  • Experience in customer service and sales

Requirements:

  • Answers phones from referral sources for intakes
  • Enters telephone referral information into EMR
  • Obtains initial authorization for care from insurance company
  • Refers to scheduling department need for coverage
  • Reports back to referral source status of start of care
  • Documents and reports to Supervisor when there is a change in client status, change in service requirement, change in employee assigned to case and any unsatisfactory performance
  • Is productive and uses time efficiently
  • Other duties as required
