Nursing Supervisor – Nurse Practitioner

Job Description

We are seeking a Nurse Practitioner for a full-time, salaried position on Long Island for our Private Pay Division.

The nursing supervisor will partner with our experienced healthcare recruiters, to assess those who are in the hiring process, hire will be provided appropriate training and educational resources for our field staff to succeed and provide a higher level of skilled nursing within the homes.

Responsibility of nursing supervisor:

  • Following New York State Dept of Health regulations for Licensed Home Care Agencies
  • Completion of initial patient referrals
  • Completion of initial patient admission visits for assessment and development of appropriate plan of care
  • Supervision of paraprofessional personnel to ensure provision of high-quality services
  • Engages patient and/or family in addressing patient care needs
  • Oversee staffing coordination of cases to ensure that appropriate personnel are staffed to meet patient needs.
  • Coordination with Insurance Company with Patient Status Update as Requested
  • Periodic Home Visits as per DOH Regulations for Re-evaluation of Patient Status, Recertification,
  • Ensure timely completion of Required Electronic Health Documentation
  • QAPI Activities to Ensure Compliance with Quality and DOH Regulations
  • Participate in Development of policies and procedures
  • Ensure timely completion of personnel Performance appraisal
  • Risk Management activities to include incident and complaint investigation
  • Ensure compliance with Infection Control and Safety Policies and Procedures
  • Ensure coverage for Supervision Activities as needed to meet the needs of the Care Plan
  • Hiring, Disciplinary and Termination of Employees as needed
  • Identification of educational needs with development of Training Program as needed
  • Business Development Activities as needed
