Physical Therapist (PT)

Job Description

The PT is an important member of the Always Compassionate Home Care Agency, Inc. nursing service team. Establishes treatment plans and treats patients using a variety of methods, including exercises, stretching maneuvers, hands-on therapy, and equipment to ease patients’ pain and help them increase their mobility. Being a Physical Therapist – Home Care works cooperatively with physicians, case managers, and adjustors.

Qualifications:

  • Current registration/license
  • Minimum of one (1) year’s clinical experience.

Responsibilities:

  • Develop, document and implement rehabilitation plans that specify therapeutic interventions distinctive to patient’s needs.
  • Treatments, as prescribed, are given to relieve pain and/or develop or restore functions.
  • Participation in development of patient plans of care as per policy and procedure.
  • Instructing patient and family/caregiver in order to ensure an effective overall physical therapy program for the patient.
  • Maintain safe environment.
  • Maintain documentation of all activities and ­observation according to the requirements of the individual health care setting. Keep documentation accurate and informative, address skilled needs.
  • Promotes education for patients, patient’s families and the community.
    1. Identification of patient, patient families and community education needs is demonstrated
    2. Educational programs are developed and implemented to address patient, patient’s family and community needs
  • Participates in personal professional growth and development
    1. Participation in education programs is consistently demonstrated
    2. Review of current literature regarding Home Health Care and Health Care is demonstrated
  • Documents and provides Home Exercise Program (HEP) in patient’s home with a copy of the HEP given to Nursing Supervisor Staff.
  • Immediately reports any changes in the patient’s condition or incidents to Nursing Supervisory staff
