Job Description

The Registered Professional Nurse (RN) is an important member of the Always Compassionate Home Care Agency team. She/ He are a part of a professional group tending to the physical and psychosocial needs of the patient and his/her family. The RN is one who shows compassion while treating patients with dignity, responds to actual or potential health problems, performs health teaching, provides supportive care. It’s your job to evaluate medical care, answer patient questions and communicate any/all concerns to your nurse supervisor.

Qualifications:

Graduate of an approved nursing curriculum with current NY state licensure as a RN.
A requirement of two (2) years Home Care experience is needed to meet NYS Department of Social Services regulatory requirements.

Responsibilities:

Collect patient health histories and perform an assessment of patient’s referred to us.
Observing and record patient behavior.
Assist with development and implementation of the nursing care plan (physical and psychosocial intervention, etc.).
Maintain safe environment.
Orient and supervise HHA per visit frequency
Participate in patient/family teaching activities and counseling of patient/families when necessary.
Maintain documentation of all nursing activities and observation according to the requirements of the individual health care setting. Keep documentation accurate and informative, address skilled nursing needs.
Monitor, record and report symptoms or changes in patient’s condition.
Safety and standard precautions to be followed at all times.
HIPAA regulations to be followed per agency policy and federal/state regulations.
