Home Care Registered Nurse (RN)

Job Description

As a 1:1 Home Care Nurse (RN), you will play a critical role in providing compassionate and high-quality healthcare to patients in the comfort of their homes. You will work closely with patients and their families to administer treatments, monitor patient progress, and offer emotional support. Your dedication and expertise will contribute to improving the overall well-being and quality of life of our patients.

Key Responsibilities:

Medication Administration:

  • Administer prescribed medications and treatments accurately and safely.
  • Educate patients and their families on medication regimens and potential side effects.

Monitoring and Documentation:

  • Continuously monitor patients’ vital signs, symptoms, and progress.
  • Maintain detailed and accurate patient records, including medical history and treatment plans.
  • Report any significant changes or concerns to the physician and healthcare team promptly.

Patient Education:

  • Provide patients and their families with education on managing their conditions, promoting self-care, and preventing complications.
  • Offer emotional support and counseling to patients and families.

Advanced Care:

  • Daily activities may include ventilator and tracheostomy management, medication administration, g-tube feedings, and other medical procedures as required.
  • Ensure infection control practices and safety protocols are followed diligently.

Salary: $42/hr RN

Job Requirements:

  • Current NYS Registered Nurse (RN) License & Registration
  • Active BLS Certification
  • Up-to-Date Physical Health Assessment
  • PPD, Chest X-Ray, or QuantiFERON Gold TB Blood Results
  • MMR/Titers Lab Report
  • Two Forms of Government Issued ID
